Dear South Asian community member:
I am writing to request your assistance in helping health professionals understand some of the unique health issues and concerns that South Asians face in the United States. By understanding these issues, I hope to be able to increase awareness about the importance of South Asian health and secure resources to prevent disease and promote health among our important population.
The following link takes you to a survey that will take approximately 15 – 20 minutes to complete. It will not ask you about any identifying information, only questions about some of your beliefs and behaviors. For your time and effort in completing the survey, you will be entered in a drawing for a $250.00 gift card from the Apple store. Once you finish, you will be taken to another site where you can simply put in any email address that you are able to access to be considered for the raffle prize.
Please click on this link to be taken to the survey: https://www.surveymonkey.com/s/SCSAFBO052312
In addition, please feel free to forward this email to any South Asian friends, colleagues, or family members who might be interested in helping us better understand the health issues of South Asians.
I thank you for taking the time to consider this request and hope that you do take the survey to help us understand how to improve the health of our community.
Sincerely,
Dr. Arnab Mukherjea
