Video Permission Forms

Before you record your classroom instruction, you must ensure that you have appropriate permission from the parents/guardians of your students who appear in your recording (as well as from any adults who appear in the recording).

Please review Pearson’s Guidelines for Video Confidentiality for Candidates. Remember that you must mask or remove any names that appear on your submitted materials and use students’ first names only in your video clips. You may not display the video publicly in any way.

Each school system has its own policy and form to secure permission for video recording. Select the county in which your school is located.

Have the principal at your school sign the following form. (Please check the folder of signed forms or with your supervisor before doing this, as the principal may have already signed this form for you!) Then, copy and distribute the form to the students in the class in which you will complete edTPA.

Student Release Form – Anne Arundel County

Create a chart for yourself to keep track of which students may appear in your video clips and which may not. Students who do not return permission forms may not appear in your video clips.

You will scan and upload the signed permission forms to Tk20 for storage. Instructions can be found here.

Have the principal at your school sign the following form. University supervisors and/or interns should distribute the following memo to mentor teachers at the beginning of the semester. Mentor teachers will need to provide information on which students in the class have opted out of appearing in videos and/or of having their work samples used.
